This Data Usage Agreement entitles Faithlife SK, LLC  and its agents and associates which may include Help Desk Agents and/or Developers access to and use of the Servant Keeper database belonging to the organization whose name is entered in the form below. The data will be used for testing purposes.  The data will not be sold or used for any other purpose other than testing issues the signer may have with any of their Faithlife SK, LLC products.

Faithlife SK, LLC, Inc shall use all appropriate safeguards to protect the data while using it for testing purposes.  No data will be copied or reproduced in any form or manner except for testing purposes.

By completing this form and clicking submit you are digitally signing this agreement and taking an oath that you have the authority to grant such access on behalf of the organization indicated in the form below.
